Widely recognized for our care management expertise, it is no surprise that Monroe Plan’s Health Home Care Management Agency (HH CMA) team were invited to present at the 2023 New York State Care Management Coalition – 2023 Annual Training Conference this past June. Leslie Trezise, SVP, Health Care Services and Donna Crane, Manager, HH co-presented on “Safety in the Community”. The presentation was well received by all. The Coalition provides quality, state-of-the art educational programming, information, and networking opportunities on best practices in the care management field. Monroe Plan is pleased to be an active member, presenter, and board representative for the Coalition!
