Monroe Plan for Medical Care was selected by the New York State Office of Mental Health to implement the Safe Options Support (SOS) program in New York State’s Southern Tier Region covering Steuben and Chemung counties and Central Region covering Onondaga and Oswego counties. SOS teams will perform homeless street outreach and work closely with shelters, housing programs, healthcare providers, and other community organizations to ensure unhoused individuals receive the full spectrum of care, services, and support they need to achieve and maintain housing stability.

Monroe Plan has been dedicated to breaking down barriers to good health and improving the quality of life for underserved populations for over 50 years. We are proud to serve this Mission through the Safe Options Support (SOS) program.

What is the Safe Options Support Program?

The Safe Options Support (SOS) Program helps people who are homeless, whether on the street or in temporary shelters, by connecting them to services that address their immediate needs and support their transition to stable housing. The SOS team uses a person-centered and trauma-informed approach to meet individuals where they are in the community. They follow a case management model called Critical Time Intervention, which is designed to help vulnerable individuals during difficult transitions. The team includes licensed behavioral health clinicians, case managers, and peer specialists.

How can a Safe Options Support Manager help me?

  • Outreach and canvassing to find individuals in need

  • Immediate help with essentials like food, water, clothing, blankets, and hygiene supplies

  • Connecting individuals to resources like legal services, job assistance, financial counseling, education, and social support services

  • Linking individuals to mental health, substance use treatment, and medical care

  • Helping ensure continuity of care before and after hospital stays

  • Assisting with housing applications, interviews, and securing transitional or permanent housing

  • Providing health education and coaching on hygiene and wellness

  • Helping with identification documents, benefits like Medicaid, SNAP, and Social Security

 We are currently serving Steuben, Chemung, Onondaga, and Oswego.
